The National Transportation Safety Board has sent a team of expert investigators to examine a Southwest Airlines Boeing 737 after a woman was killed by an apparent mechanical failure. When the aircraft, which was en route from New York to Dallas, blew an engine, flying shrapnel smashed a window, causing the cabin to suddenly decompress, upon which a female passenger was partly suck through the broken window. Although fellow passengers managed to pull her back into the cabin, Jennifer Riordan later died due to her catastrophic injuries. Seven other passengers suffered minor injuries in the incident.

According to preliminary reports, the engine appears to display signs of “metal fatigue”, with some of the fan blades separated from the main body. The full investigation, during which the entire engine will be examined thoroughly to identify why the accident happened, could potentially take up to 15 months to complete. Southwest Airlines have announced that they will be carrying out their own safety checks on all similar aircraft over the coming weeks, to ensure the same issue does not exist elsewhere.
